Inflammatory bowel disease in underserved populations: lessons for practice

Abstract: Purpose of reviewThe incidence of inflammatory bowel disease (IBD) is increasing in minority groups across the USA. There are racial and ethnic disparities in IBD care and outcomes that are rooted in historical injustice and inequities in the social determinants of health.Recent findingsCurrent literature has identified racial, ethnic and sociodemographic disparities in therapeutics and outcomes for IBD, including disease severity, morbidity and mortality.SummaryStrategies to achieve equity in IBD include tack… Show more

“…Nearly one-quarter of all Asian CD patients (0.23, 95% CI: 0.17–0.29) and one-third of Black CD patients (0.31, 95% CI: 0.11–0.52) had perianal disease compared to Caucasian CD patients (0.14, 95% CI: 0.12–0.16) (P=0.02). Similar results were found in a study from Odufalu et al which was a literature review that found AA had a higher proportion of penetrating disease, perianal disease, and extraintestinal manifestations of uveitis, arthralgias and ankylosing spondylitis compared to the Caucasian populations ( 20 ).…”

“…For example, nearly 200 population-based cohort studies were included in meta-analysis examining disparities in clinical phenotypes for adult patients with IBD. 10 Findings revealed that NHB patients compared with NHW, Hispanic, and Asian patients, had a significantly higher proportion of penetrating disease and were less likely to have nonpenetrating disease than all other racial groups. Similarly, findings in pediatric patients with IBD reveal that NHB children (51.3%) present with higher proportions of structuring and penetrating disease than NHW children (27.4%).…”

“…9 However, a population-based cohort study from 1970 and 2010 concluded that while IBD incidence increased by 39% for NHW Americans, it increased in NHB individuals by an alarming 134%. 10 Despite this disconcerting increase in IBD prevalence among NHB individuals, there has been limited research into treatment and outcomes in this vulnerable IBD population. 11 Access to care, disease assessment, clinical trial enrollment, and evaluation of environmental factors have all been prioritized for future research in this population.…”

“…In patients with inflammatory bowel disease (IBD), socioeconomic-based disparities can lead to poor patient outcomes, including increased disease severity, diagnostic delays, poor appointment and medication adherence, or increased hospitalizations and death. 8 , 9 …”
