Inflammatory bowel disease related osteonecrosis: report of a large series with a review of the literature

Abstract: Summary Background : Osteonecrosis is a major complication of inflammatory bowel disease usually associated with steroid use. There are few large series available detailing the specifics of affected patients. Aim : To identify any specific characteristics of osteonecrosis in this cohort. A major focus was placed on steroid dose, the average time between diagnosis of IBD and appearance of osteonecrosis and the frequency of multiple joint involvement. Methods : Our study identified 23 patients in the practices o… Show more

“…Th e diagnosis is generally not suspected until a patient complains of specifi c joint pain, and it is then established by magnetic resonance imaging. Th e distribution of aff ected joints in IBD is similar to other conditions associated with osteonecrosis, with the hips being the most frequently involved ( 141 ).…”

“…More profound psychiatric problems, including depression and psychosis, may become unmasked or exacerbated by steroid therapy. Osteonecrosis, particularly of the femoral heads, can be related to corticosteroid therapy or the underlying disease [458,459].…”

“…However, today it is recognized that IBD predispose patients to AN. Some studies suggest that IBD lowers the threshold of corticosteroids (lower doses per day and for shorter time) needed to induce AN (2,5). Also, AN may occur in patients with IBD who have not received any corticosteroid therapy (1) suggesting that other pathogenic factors must be involved, such as hypercoagulability (5) or parenteral nutrition with infused lipids (7).…”
