Inflammatory Markers in Patients after Hematopoietic Stem Cell Transplantation

Abstract: Infections are one of the most common complications after hematopoietic stem cell transplantation (HSCT). Diagnosis is established by analysis of clinical symptoms and results of diagnostic tests such as biochemical panels, microbiological cultures, and visual diagnostics. As the microbiological cultures yield positive results in only some patients and visual diagnostics might miss the infectious source, the diagnosis and proper treatment often depends on clinical assessment supported by laboratory test result… Show more

“…Thymoglobulin consists of a polyclonal mix of antibodies directed toward T lymphocytes and induces complementdependent lysis and Fc receptor-dependent opsonization mediated by monocytes and phagocytes [28]. Thymoglobulin infusion can induce an inflammatory response with high cytokine levels and fever, chills, and a rise in CRP (serum sickness) [29][30][31]. Thymoglobulin is also thought to have immunoregulatory effects [32].…”

“…The negative microbiological cultures may result from aggressive prophylaxis and antiinfective therapy with antibiotics. Furthermore, a sterile laminar-flow environment, the presence of agranulocytosis, medication for infection and GVHD may all have an impact on test results and mislead physicians (13); thus, the importance of a timely diagnosis of infection cannot be over-emphasized (14).…”

“…21 Increased serum level of some proinflammatory cytokines was also observed during such complications after transplantation. 22 In Iran, the incidence of transplantable diseases is relatively high, reaching 1,400 patients per year. 23 It was reported that a total of 3,170 HSCTs were carried out since the establishment of transplantation centers in Iran within the past 20 years, and the rates are increasing annually.…”
