Inflammatory myopathies after allogeneic stem cell transplantation

Abstract: Among recipients of allo-SCT, weakness is common, and the incidence of IM is increased. Histopathological diagnoses are varied, and we report findings of necrotizing myopathy and anti-MDA5-associated DM. Muscle Nerve, 2018.

“…8,13 Given the common finding of muscle fiber necrosis and regeneration, GVHD-related myositis has also been anecdotally labeled as a necrotizing myopathy. 14 Our series supports this wide spectrum of histopathological findings in a single We also highlight the macrophagic-predominant infiltration, rarely reported in GVHDrelated myositis. 14,15 While historically, T-cells were considered the key player in the pathogenesis of GVHD, there is growing evidence underlining the essential role of macrophages especially in skin and lungs, and here we show similar findings in muscle.…”

“…GVHD can affect numerous organ systems, including muscle tissues. Furthermore, GVHD‐related inflammatory myopathy has similar clinical characteristics to autoimmune inflammatory myopathy, infiltrated by cytotoxic T cells; the prognosis of GVHD is, however, good 18 . Persistent and slow‐replicating viruses, such as retrovirus HTLV‐I, may be possible factors potentially triggering IBM's autoimmunity 19 .…”
