Influence of autogenous platelet concentrate on combined GTR/graft therapy in intra‐bony defects: A 13‐year follow‐up of a randomized controlled clinical split‐mouth study

Abstract: Within the limits of this study, the data show that most of the CAL gain following GTR can be maintained over 13 years. The additional use of APC had no positive influence on the long-term stability.

“…The median V-rAG was found 66.7% (in situ) and 60.0% (extracted) between baseline and 1 year, and 57.1% in the in situ-group between baseline and the follow-up after up to 26 years.4 | D ISCUSS I ONThere are only very few studies reporting on long-term outcomes after regenerative periodontal therapies, as it was recently concluded byWu et al (2017) in their systematic review. With regard to GTR (or combined GTR/graft therapy) in intra-bony defects, there are only five studies with follow-up periods of 10 years (Nickles, Ratka-Krüger, Neukranz, Raetzke, & Eickholz, 2009; Nygaard-Østby, Bakke, Nesdal, Susin, & Wikesjö, 2010; Pretzl, Kim, Holle,& Eickholz, 2008;Pretzl et al, 2009;Sculean et al, 2008), one study with 13-year follow-up evaluating additional application of autogenous platelet concentrate(Cieplik et al, 2018) and two studies with follow-up periods of 20 years(Cortellini et al, 2017;Petsos et al, 2019). These latter two studies are prospective clinical studies comparing GTR therapy and open flap debridement (OFD) but comprise only 41 defects (in 41 patients; parallel design;Cortellini et al, 2017) or 38 defects (in 12 patients; split-mouth design;Petsos et al, 2019), respectively.…”

“…These included scaling and root planning, 97,98 osseous surgery, 97,98 flap surgery combined with laser conditioning, 98 and for the majority of the cases, regenerative techniques, using a wide variety of biomaterials. 8,[60][61][62]64,[99][100][101][102][103][104]107,109,110,112,115 An interesting finding is that, in all but 1 study in the tooth retention group, there was a CAL gain and PD mean values improved with time. Only Orsini et al reported a slight increase in PD mean values in the follow-up examination compared to the baseline.…”

“…After discussion and further evaluation between the authors another 35 full-text articles were excluded and not analyzed further (Table 1). 1,[19][20][21] Twenty studies reporting on tooth preservation and periodontal treatment or regeneration 8,9,[60][61][62][63][64][98][99][100][101][102][103][104][105][106][107][108][109][110][111][112][113][114] and 3 studies reporting on tooth extraction followed by implant placement were finally selected for inclusion in the present review. 22,113,114 These were 11 RCTs and 13 prospective studies.…”
