BackgroundImplant and superstructure provide a complex system, which has to withstand oral conditions. Concerning the brittleness of many ceramics, fractures are a greatly feared issue. Therefore, polymer-infiltrated ceramic networks (PICNs) were developed. Because of its high elastic modulus, the PICN crown on a one-piece zirconia implant might absorb forces to prevent the system from fracturing in order to sustain oral forces. Recommendations for the material of superstructure on zirconia implants are lacking, and only one study investigates PICN crowns on these types of implants.Accordingly, this study aimed to examine PICN crowns on one-piece zirconia implants regarding bond strength and surface wear after long-term chewing simulation (CS).MethodsTwenty-five hybrid ceramic crowns (Vita Enamic, Vita Zahnfabrik) were produced using computer-aided design/computer-aided manufacturing (CAD/CAM) technology and adhesively bonded (RelyX™ Ultimate, 3M ESPE) to zirconia implants. Twenty of the specimens underwent simultaneous mechanical loading and thermocycling simulating a 5-year clinical situation (SD Mechatronik GmbH). Wear depth and wear volume, based on X-ray micro-computed tomography volume scans (Skyscan 1172-100-50, Bruker) before and after CS, were evaluated.All crowns were removed from the implants using a universal testing machine (Z010, Zwick GmbH&Co.KG). Subsequently, luting agent was light microscopically localized (Stemi 2000-C, Zeiss).With a scanning electron microscope (SEM, Phenom™ G2 pro, Phenom World), the area of abrasion was assessed.Results After CS, none of the tested crowns were fractured or loosened.The maximum vertical wear after CS was M = 0.31 ± 0.04 mm (mean ± standard deviation), and the surface wear was M = 0.74 ± 0.23 mm3.The pull-off tests revealed a 1.8 times higher bond strength of the control group compared to the experimental group (t(23) = 8.69, p < 0.001).Luting agent was mostly located in the crowns, not on the implants.The area of abrasion showed avulsion and a rough surface. ConclusionsPICN on one-piece zirconia implants showed high bond strength and high wear after CS.

The aim of the study was to evaluate the capacity to resist fracture in different core buildup materials with porcelain fused to metal (PFM) crown. Totally, 45 mandibular single rooted first premolars were collected, which were sound along with similar shape and size. The teeth were sectioned at 15 mm above the root apex sparing the sound tooth structure. The teeth were endodontically treated with the crown-down technique using nickel-titanium (NiTi) instrumentation. The specimens were randomized into three groups as per the core materials used and were labeled accordingly. Group I consisted of dual-cured composite resin, group II consisted of glass ionomer reinforced with resin, and group III consisted of Miracle mix. Universal loading machine is used for measuring the compres-sive load applied to fracture the tooth. The mean value of compressive strength was maximum in the dual cured composite resin (598.42 ± 22.64) followed by glass ionomer reinforced with resin (478.88 ± 26.74) and Miracle mix (442.16 ± 30.10). The results showed a significant difference statistically within the core materials used with p < 0.05. The results from the Tukey's test of multiple comparisons between dual-cured composite resin glass ionomer reinforced with resin, dual-cured composite resin Miracle mix, and glass ionomer reinforced with resin Miracle mix showed a highly statistical difference with p < 0.05, which is significant. This study showed that the dual-cured composite resin had maximum resistance to fracture compared with other core buildup materials on teeth which were endodonti-cally treated. Restoration of a tooth which is structurally compromised is a tricky job for all the dentists. Restoring it with a proper core buildup material with adequate fracture resistance makes the tooth structure stable. The core material should be able to resist all types of occlusal forces and to distribute it equally within the tooth structure. Teeth that have become severely compromised by decay, large failing restorations, or fracture most often require full-coverage crowns to restore them to their original form and function. A core buildup is a restoration placed in a badly broken down or grossly decayed tooth to restore the bulk of the coronal portion of the tooth. This facilitates the subsequent restoration by means of an indirect extra coronal restoration. The strength of the tooth can be enhanced with intracoronal support. It should have compressive strength to resist intraoral forces and flexural strength to prevent core dislodgement during function. It should also have sufficient flexural strength, biocompatibility, resistance to leakage of oral fluids at the core-tooth interface, ease of manipulation, and ability to bond to remaining tooth structure. The primary purpose of a core build-up is to replace enough missing tooth structure to permit the creation of ideal retention and resistance form in the crown preparation. Materials used for core restoration after endodontic treatment include amalgam, resin composites, zirconia to name a few. The aim of this review is to evaluate the use of zirconia as a core material by assessing the studies available in comparison to other core materials.
