Influence of genetic and non-genetic factors on acenocoumarol maintenance dose requirement in a Tunisian population

Abstract: The PA demonstrated that VKORC1 and CYP2C9 polymorphisms contribution was more important than clinical factors. Applying the PA would allow dose adjustment to treat patients in a personalized manner.

“…Based on these findings, we can hypothesize that the ancestral "C" allele is a protective allele for the Tunisian patients against hemorrhagic complications caused by an overdose of acenocoumarol. Our results oppose those of Ajmi et al who declared the protective effect of rs7294 "T" allele against bleeding caused by acenocoumarol treatment 53 . This study was conducted on 246 Tunisian patients who originated from the Sahel region.…”

“…VKAs have a narrow therapeutic range and, therefore, levels outside this range increase the risk of thromboem-bolism or bleeding incidences.VKAs are drugs that interact with various medications and foods [4][5][6] and maintenance doses of VKAs are affected by patient age, 7) body weight, 8) clinical conditions, 9,10) and genetic factors. 8,11) According to the Clinical Pharmacogenetics Implementation Consortium (CPIC) guidelines, cytochrome P450 2C9 (CYP2C9), vitamin K epoxide reductase complex subunit 1 (VKORC1), and cytochrome P450 4F2 (CYP4F2) are known to influence the warfarin dosing algorithm. 11) Similarly, the maintenance dose of acenocoumarol is also affected by VKORC1 and CYP2C9 genes.…”

“…Majority of the patients with these variant genotypes were found to belong to a low acenocoumarol dose group [27]. In Tunisia, Ajmi et al demonstrated an association between daily acenocoumarol dose and CYP2C9 (*2, *3) and VKORC1 (H1, H7) haplotypes [28].…”
