Influence of hemorheological factors on the development of retinal vein occlusion

Abstract: Quick diagnosis and therapy create a possibility for successful treatment. Corticosteroid positive influence on visual acuity improvement has been indicted in two randomized, double-blind controlled studies - CRUISE and BRAVO. In both studies, the improvement of vision has been accompanied by a significant reduction of oedema in the vicinity of macula, reflected in the central retinal thickness.

“…Fifty-five patients (61.1%) had hypertension, while 14 patients (15.6%) had type 2 diabetes but no diabetic retinopathy (DR). The mean Hb level was 160±25.8 g/L (range, 82-255 g/L), while the mean RBC count was (5.4±0.8)×10 9 /L (range, 3.9-9.1). The mean Hb level of the men was significantly higher than that of women.…”

These changes may account for the higher incidence of RVO in TAR [9] . On the other hand, there is growing evidence on association between systemic adverse events (SAEs) related to anti-VEGF therapy (cerebral infarction, myocardial infarction, etc.)…”

“…Increased viscosity of blood creates the risk of CRVO and hemostasis abnormalities promote the development of neovascularization. 12,13 In retinal venous thrombosis, corticosteroids reduce macular oedema, inhibit expression of endothelial growth factor (VEGF), 14,15 reduce the formation of mediators of cystic macular oedema, 16,17 stabilize the blood-retinal barrier and improve hemorheological parameters in retinal vessels. The administration of corticosteroid directly to the vitreous chamber allows to achieve the optimal therapeutic concentration in the eye tissues.…”

“…To reveal the pathophysiology of different types of RVO, investigation of systemic conditions combined with RVO may provide useful clues. Thus far, the development of RVO has been associated with obesity, smoking, and hemorheological factors [46]. Systemic conditions have been also reported as risk factors, such as hypertension, diabetes, dyslipidemia, and other cardiovascular diseases [7].…”
