Influence of hospitalization on prescribing safety across the continuum of care: an exploratory study

Abstract: BackgroundTransitions between different levels of healthcare, such as hospital admission and discharge, pose a considerable threat to the quality and continuity of drug therapy. This study aims to further explore the current role of hospitalization in prescribing error exposure and medication-related communication as patients are transferred from and back to ambulatory care.MethodsAssisted by electronic decision support, pre-admission and discharge medication regimens of 187 adult patients in a German universi… Show more

“…Another aspect of the transition between hospital and community care concerns treatment changes at each step; these changes occur for more than 95% of hospitalised patients [11]. They can induce misunderstandings for patients, incomplete information and communication at transition steps, and thus induce medication errors and decreases in medication safety [12,13].…”

“…21 % der Patienten erhalten mindestens ein nicht korrekt dosiertes bzw. kontraindiziertes Medikament [13]. Die Durchführung einer bestmöglichen Medikationsanamnese ermöglicht die umfassende und strukturierte Erfassung und Evaluation der Medikation zum Zeitpunkt der Aufnahme im Krankenhaus [14].…”

“…Indeed, up to 10% of all hospital readmissions occur as a consequence of ADE, and nearly 1 in 4 of these ADE is caused by drugs just started during the index hospitalization [1,3-5]. Since during hospitalization, more than 95% of all prehospital drug therapies are modified, appropriate follow-up monitoring is particularly important [6-9]. Furthermore, in the discharge medication, over half of the drugs are newly prescribed during hospitalization, emphasizing the need for closer monitoring during the initial postdischarge phase [8].…”
