Influence of Labour on the Lecithin, Lecithin/ Sphingomyelin (L/S) Ratio and Palmitic Acid Values in the Amniotic Fluid

Abstract: The lecithin/sphingomyelin (L/S) ratio, lecithin concentrations and palmitic acid levels in amniotic fluid were studied during labour and compared with values found before labour. All were significantly higher in women in labour. The explanation and significance of this finding are briefly discussed.

“…They found these values were much higher in those born vaginally than by elective CS. Cabero 11 et al ., in 1976, measured amniotic fluid lecithin/sphingomyelin ratios, and concentrations of lecithin and palmitic acid in a group of mothers before and during labour. At every week of gestation from 34 to 43 weeks, levels were twice as high in those with labour.…”

“…The protective effect of labor is more apparent at 37-38 weeks than at 31-33 weeks (312), presumably because labor stimulates surfactant release and insufficient surfactant is stored at the earlier gestational age. Recent studies have shown that labor increases the L/S ratio and PC content of human amniotic fluid (201,(313)(314)(315). Animal studies have also shown that labor increases surfactant production (70,199).…”

“…Despite these clinical observations, the effect of labor on surfactant production had not been examined. Re-cently, however, two groups (12,13) reported that both the concentration of phosphatidylcholine and the phosphatidylcholine/sphingomyelin ratio in amniotic fluid, indexes of fetal lung maturation (14), were increased during labor in humans.…”
