Influence of Successful Chronic Hepatitis C Virus Treatment with Ledipasvir/Sofosbuvir on Warfarin Dosing Requirements in Four Veterans

Abstract: To our knowledge, this is the first case series describing a possible drug interaction between warfarin and ledipasvir/sofosbuvir (with or without ribavirin). Close monitoring is warranted when ledipasvir/sofosbuvir is initiated in patients receiving anticoagulation therapy with warfarin, especially those with evidence of cirrhosis prior to treatment. This is particularly important in the first month after starting treatment and the first month after completion. Failure to monitor and achieve therapeutic INR a… Show more

“…2,3 It has been speculated that HCV eradication could improve liver function, leading to a decreased warfarin dose-response. 1,4 If true, such a mechanism would result in persistent reduction of warfarin dose-response beyond antiviral treatment completion. If DDI is the only cause, the warfarin dose-response would eventually return to baseline after HCV treatment completion.…”

“…Pathophysiologically, cell-mediated immunity is responsible for the destruction of epithelial cells. Macrophages and CD8 T lymphocytes release a large number of cytokines that lead to the inflammation and subsequent death of epithelial cells [ 9 ].…”
