Influence of Thyroid Dysfunction on Drug Pharmacokinetics

Abstract: When thyroidfunction is altered there are a series of physiological changes which are likely to affect drug absorption, metabolism or excretion, and altered pharmacokinetics have been described for a variety of drugs in both hypothyroidism and hyperthyroidism.Absorption of riboflavine is increased in hypothyroidism and decreased in hyperthyroidism due to changes in gut motility. Opposite/in dings have been described for ethanol in rats, but remain to be confirmed in man. There is definite evidence of induction… Show more

“…In general, few studies have considered the drug absorption process in thyroid disease (Shenfield, 1981). However, an increase in intestinal transit time, which is a pathophysiological consequence of hypothyroidism, might be expected to accelerate the rate of absorption.…”

“…It has clinical implications in that the dosage & duration of anticoagulants like heparin, warfarin need to be adjusted in hypothyroid patients. For optimal anticoagulation, dosage of warfarin need to be increased than usual [34,35] . Increased fibrinolytic activity observed in hypothyroidism was normalised with thyroxine treatment.…”

“…Через несколь-ко часов могут быть введены следующие дозы про-пранолола -2-3 мг в течение 10-15 мин [25]. Назначаемые дозировки значительно варьируют в зависимости от клиренса препарата [26]. Особенности ведения пациентов с сердечной недостаточностью, нарушениями ритма сердца будут рассмотрены ниже.…”
