Ingrowing toenails: The results of treatment

Abstract: Completed questionnaires were received from 208 patients on whom 245 operations for ingrowing toenails had been performed at King's College Hospital. Avulsion of a strip of nail had an 83 per cent recurrence rate, total nail avulsion 70 per cent, Zadik operations 28 per cent and wedge resections 29 per cent. It is recommended that immediate avulsion of ingrown toenails should not be a routine practice.

“…Conventional surgical treatment of ingrown toenails has been associated with an unacceptably high recurrence rate, substantial postoperative pain, and poor cosmetic results. 10,11 Simple nail edge excision has a recurrence rate of 39%, 10,12 and total nail avulsion has a recurrence rate of 83%, 13,14 whereas soft tissue resection has a cure rate of 60%. 10,14,15 The reported recurrence rate after wedge resection ranges from 12% to 30%.…”

“…Nail extraction or partial nail extraction alone has a high recurrence rate, ranging between 64% and 83%. 7,[14][15][16] The recurrence rate of wedge resection with incision of the eponychium ranges from 10% to 30%. 5,6,8,14,15,17,18 A disadvantage of the wedge resection is the incision of the eponychium, which may lead to prolonged healing or problems with the scar.…”

“…11 Abnormal transmission of forces while walking may lead to abnormal transmission of pressure through the nail into surrounding soft tissues. 7 This needs to be addressed by supportive care which includes proper cutting of nails exactly perpendicular to long axis, use of footwear with broad forefoot and use of toe distractors to alter lines of transmission of force.…”
