Inhibitors of the renin–angiotensin system: The potential role in the pathogenesis of COVID-19

Abstract: Coronavirus disease 2019 (COVID-19), which initially began in China, has spread to other countries of Asia, Europe, America, Africa and Oceania, with the number of confirmed cases and suspected cases increasing each day. According to recently published research, it was found that the majority of the severe cases were elderly, and many of them had at least one chronic disease, especially cardiovascular diseases. Angiotensin-converting enzyme inhibitors/angiotensin receptor blockers (ACEIs/ARBs) are the most wid… Show more

“…Our results support the administration of anticoagulant drugs at proper doses in all COVID-19 patients, as also suggested by some guidelines (37)(38). The possible beneficial, harmful or neutral role of ACEI or ARB remains a matter of debate, and larger studies are probably needed (39,40).…”

“…Cardiovascular morbidity has also been associated with both worse outcomes in COVID-19. Whether the excess of mortality in patients with myocardial injury can be explained by the higher prevalence of comorbidities in this population is still a subject of discussion [4,[9][10][11][12][13][14].…”

“…ACE2 is mainly present in the lungs and small intestines and it converts Ag II to Ag (1 -7), which binds to Mas receptor producing anti-inflammatory and anti-fibrinolytic activities. On the other hand, activation of Ag II receptors by Ag II results in proinflammatory and pro-fibrinolytic activities [14]. As discussed earlier, ACE2 is the main host receptor responsible for the internalization of SARS-CoV-2 through interaction with the S protein on the viral cell surface [4].…”

“…The RAAS starts with renin cleaving angiotensinogen to Ag 1 which is further cleaved by ACE to Ag 2. Ag II binds to Ag II receptors 1 and 2 [14].…”
