Initiation of Warfarin Treatment in Outpatients with Nonrheumatic Atrial Fibrillation: A Scheme for Early Indication of Maintenance Dose

Abstract: Eighty-four outpatients with nonrheumatic atrial fibrillation starting oral anticoagulant treatment were adminis tered warfarin (10 mg/day) for the first 2 days of treatment. Prothrombin time, expressed as International Normalized Ratio (INR), was measured on day 3 and plotted against the weekly warfarin maintenance dose, defined as the dose able to maintain INR between 2.0 and 3.0 (therapeutic range) on three consecu tive occasions, 1 week apart. The data fit a regression line (r = 0.74, p < .0001) that we… Show more

“…Patients may not take dosages appropriately after discharge and may not understand instructions appropriately. The initiation of warfarin is traditionally difficult despite the presence of a number of management protocols [10–12]. The trend to early discharge of patients with embolic disorders after the initiation of warfarin makes the transfer of information to general practitioners (GPs) very important.…”

“…On a previous observation, we found that a 10-mg protocol for 2 consecutive days resulted in excessive anticoagulation by day 3, especially in elderly patients. 25 We therefore switched to a 5-mg protocol for 4 days that resulted in a more gradual increase of the INR values. Interestingly, we found a correlation between the INR achieved on day 5 and the expected maintenance dose.…”

“…2,3 Reports also indicate that the quality of initiation of anticoagulation in the hospital setting may be less than optimal. 4,5 Initiating warfarin is complex as it is influenced by many factors, such as underlying warfarin sensitivities, differences in vitamin K status, concomitant medications and disease states, and warfarin's delayed action. An early elevation in the international normalised ratio (INR) may not accurately reflect the antithrombotic effects of warfarin given the difference in half-lives of the vitamin K-dependent clotting factors.…”
