Insulin‐like growth factor‐1 in psoriatic plaques treated with PUVA and methotrexate

Abstract: Both methotrexate and PUVA therapy for psoriasis are associated with a decrease in PASI score and IGF-1. The IGF-1 down-regulation may possibly be a consequence of the decrease in cytokines and inflammatory cellular infiltrate that occur following treatment with either modalities or due to their effect on local fibroblast activity and proliferation.

“…13 Besides this, clinical improvement of psoriasis was found to be associated with decreased expression of IGF-1. 14 As S6 phosphorylation is not evident in dividing keratinocytes of healthy skin, S6 activation seems to be dispensable for normal keratinocyte proliferation. Thus, mTOR inhibitors may selectively target pathologically activated keratinocytes, a hypothesis supported by the observation that mTOR inhibition suppresses UVB-induced keratinocyte hyperproliferation, 15 but does not affect normal epidermal turnover.…”

“…In the skin affected by psoriasis a much higher IGF-1 production by fibroblasts and IGF-1R overexpression is observed, compared to unaffected skin [54]. Therapies with anti-psoriatic drugs such as methotrexate (MTX) or PUVA, which reduce severity of the disease, lead to a decrease in IGF-1 expression, although these levels are still higher compared to those in healthy subjects [55].…”

“…W skórze zmienionej łuszczycowo obserwuje się znacznie większą produkcję IGF-1 przez fibroblasty oraz nadekspresję IGF-1R w porównaniu ze skórą niezmienioną [54]. Terapie z zastosowaniem leków przeciwłuszczycowych, takich jak metotreksat (MTX) lub PUVA, prowadzą do zmniejszenia ekspresji IGF-1, mimo że stężenia te nadal są wyższe niż u osób zdrowych [55].…”

“…IGF-1 is reported to be one of the growth factors involved locally in the pathogenesis of psoriasis. 2 Acromegaly is found to be associated mostly with psoriasis and acanthosis nigricans, but until now, we did not know about the association with PV. It was suspicious in our case that increased levels of IGF-1 and IGFBP-3 are associated with his skin disease and normalized after his remission for PV.…”
