Integrated Care Facilitation Model Reduces Use of Hospital Resources by Patients with Pediatric Asthma

Abstract: This evaluation assessed a model of care for pediatric asthma patients that aimed to promote health and reduce their preventable and avoidable use of acute hospital services.
