Inter-hospital transfer of ECMO-assisted patients with a portable miniaturized ECMO device: 4 years of experience

Abstract: Our experience demonstrates that miniaturized, portable ECMO therapy allows location-independent, out-of-center stabilization of pulmonary compromised patients with consecutive inter-hospital transfer and further in-house treatment, so that sophisticated ECMO therapy can be offered to every patient, even in hospitals with primary healthcare.

“…Transports have been performed both with roller and centrifugal pumps. Roller pumps were more common in the earlier era of transports ( 11 14 ) and later on centrifugal pumps were preferred by most centers ( 6 , 8 , 9 , 15 18 ). Today, the latter is recommended by ELSO due to improved functionality and safety in combination with a shorter circuit ( 10 ).…”

“…Bangkok Hospital team is the fi rst transported ECMO team in Thailand and we perform this procedure with good results as at other ECMO centers. [8][9][10] The successful transportation of ECMO patients requires a skilled Transport Medical Director who is responsible for planning the mission, preparing and assembling the transport team. Furthermore, the ECMO transport team should receive the requisite training.…”

“…The length of this ECMO run made transport of this immunocompromised and pancytopenic patient necessary. Notably, ECMO circuits are increasingly portable, 22,23 and this is likely to allow for greater access to a broader range of hospital services for critically ill pediatric oncology patients in the coming years. Also in this case significant complications of therapy such as pancytopenia, profound immunosuppression, and febrile neutropenia/sepsis occurred during the ECMO run.…”
