Urinary Tract Infection (UTI) is a relatively common infection in children that can lead to serious complications such as renal scarring, hypertension and chronic renal failure. In this study we stressed the role of IL-8 in the diagnosis and differentiation between pyelonephritis and cystitis. This prospective descriptive study was conducted among 86 children one month to 14 years, who with clinical signs and symptoms of suspected urinary tract infections were referred to Ahvaz Aboozar hospital. Among 86 children studied 16 (18.6%) and 70 (81.4%) children were boy and girl, respectively. E. coli and Klebsiella were obtained from urine samples of 72 patients (83.7%) and seven patients (8.1%), respectively. The frequency of abdominal pain in pyelonephritis and cystitis groups was 10.4 and 18.4%, respectively. Vomiting in the pyelonephritis and cystitis groups was respectively 22.9 and 10.5%. The frequency of diarrhea in the pyelonephritis and cystitis groups was 4.2 and 2.6%, respectively. In general, dysuria was the most common symptom in the two groups (69.8%). Measurement of cytokines only takes a few hours and to do it is the easy and non-invasive. Based on the study system the mean IL-8/Cr for detecting pyelonephritis in level 7 has the sensitivity of 66% and the specificity of 72% and even the increased IL-8 after the initial experimental treatment can be a sign for the continued inflammation or antibiotic resistance. Obviously, in this recent case wider study must be carried out.

Urinary tract infections are common in infants and children. Pyelonephritis may result in serious complications, such as renal scarring, hypertension, and renal failure. Identification of the timing of release of inflammatory cytokines in relation to pyelonephritis and its treatment is essential for designing interventions that would minimize tissue damage. To this end, we measured urinary cytokine concentrations of interleukin-1␤ (IL-1␤), IL-6, and IL-8 in infants and children with pyelonephritis and in healthy children. Children that presented to our institution with presumed urinary tract infection were given the diagnosis of pyelonephritis if they had a positive urine culture, pyuria, and one or more of the following indicators of systemic involvement: fever, elevated peripheral white blood cell count, or elevated C-reactive protein. Urine samples were obtained at the time of presentation prior to the administration of antibiotics, immediately after completion of the first dose of antibiotics, and at follow up 12 to 24 h after presentation. IL-1␤, IL-6, and IL-8 concentrations were measured by enzyme-linked immunosorbent assay. Creatinine concentrations were also determined, and cytokine/creatinine ratios were calculated to standardize samples. Differences between preantibiotic and follow-up cytokine/creatinine ratios were significant for IL-1␤, IL-6, and IL-8 (P < 0.01). Differences between preantibiotic and control cytokine/creatinine ratios were also significant for IL-1␤, IL-6, and IL-8 (P < 0.01). Our study revealed that the urinary tract cytokine response to infection is intense but dissipates shortly after the initiation of antibiotic treatment. This suggests that renal damage due to inflammation begins early in infection, underscoring the need for rapid diagnosis and intervention.

Urinary tract infections are one of the most common bacterial infectious diseases in humans. Depending on the localization and the effectiveness of pathogenetic factors, various clinical pictures (lower urinary tract infection, pyelonephritis, asymptomatic bacteriuria) have to be differentiated. There are virulence factors of microorganisms on the one hand and defense mechanisms on the other, which influence the manifestation and the course of disease. The process of bacterial attachment to the epithelial cells of the boundary layer, the internalization and invasion of bacteria could be important for acute and chronic disease. Disturbances of local defense mechanisms, such as increased urinary glucose concentration in diabetes or variations of Tamm-Horsfall protein and defensin levels, may influence the course of infection. On the basis of microbiological and laboratory findings as well as the results of clinical and ultrasound procedures, the decision on the therapeutic strategy should be made. There are different treatment recommendations for acute uncomplicated and complicated cases as well as for chronic diseases. Future investigations should focus on effective therapeutic options for special immunocompromised patients in relation to the microbiological aspects and defense mechanisms of the host.
