Internal Arteriovenous Fistula Within a Radial Forearm Flap – A Novel Technique to Increase Femorodistal Bypass Graft Flow to the Diabetic Foot and Flap Covering Ischaemic Tissue Loss

Abstract: In a group of diabetic patients increased flow in a vascular bypass graft was achieved by an internal av-fistula within a radial forearm flap. This method is useful in selected cases with poor run off and large ischaemic lesions.

“…Studies have reported that an AV fistula of less than 1 cm in diameter steals less than 20% of the cardiac output and can be used safely. 9 In addition, venocutaneous fistulae can be performed to decrease high resistance in solely venous problems. 5 In the case presented here, the OCFF was modified to a bone flap by discarding the skin island due to vascular insufficiency.…”

“…13 fistula is less than 1 cm and the fistula accepts less than 20 percent of the cardiac output (Blaisdell et al, 1966;Tukiainen et al, 2006;Eweida et al, 2013;Horch et al, 2014). Regarding bone density and mineralization distal to an AV fistula, contradictory results are shown in the literature, ranging from an increase in bone growth (Vanderhoef et al, 1963) to decreased mineralization and augmented osteopenia (Muxi et al, 2009).…”
