Internal herniation of the abdominal wall

Abstract: Key Clinical Message A symptomatic arcuate line hernia should be considered in patients with acute lower abdominal complaints. This rare internal herniation is caused by a sharp ending of the posterior aponeurotic sheath of the rectus muscle, rather than the more common gradual delineation, and can cause strangulation or incarceration of abdominal contents.

“…ALH can be repaired without mesh as it doesn’t leave any internal defect and there is no associated weakness of the anterior abdominal wall [3]. This said most authors still prefer mesh coverage of the defects [1, 4], and we agree with Montgomery et al .…”

“…ALHs are difficult to clinically diagnose as they don’t protrude through the abdominal wall and are therefore often incidentally demonstrated during surgery or at best, on preoperative CT-scan imaging [1]. Most of the ALHs remain asymptomatic, but may sometimes cause pain, especially if the bowel is incarcerated into the hernia sac [2, 3].…”

“…Three cases occurred after abdominal surgery and were diagnosed as incisional hernias, but were diagnosed as ALH intraoperatively [ 4 , 15 , 16 ]. As previously mentioned, ALH is considered less likely to cause symptoms because of the wide hernia orifice; however, there have been cases of emergency surgery owing to incarceration [ 6 , 9 , 13 ]. Although ALH is a rare and little-known disease, these cases suggest that surgeons should be aware of the condition.…”
