Interprofessional Teamwork Innovation Model (ITIM) to promote communication and patient-centred, coordinated care

Abstract: The ITIM approach facilitates a collaborative environment in which patients and their family caregivers, physicians, nurses, pharmacists, case managers and others work and share in the process of care.
