Interstitial Lung Disease Evaluation: Detecting Connective Tissue Disease

Abstract: Interstitial lung disease (ILD) is a common manifestation of connective tissue disease (CTD) and is often associated with significant morbidity and mortality. The evaluation of ILD in patients with CTD is complex because of the heterogeneity of the CTD spectrum, the various patterns and degrees of severity of ILD encountered, and because ILD can be identified at any point in time in these patients. A thorough - and optimally multidisciplinary - evaluation is needed when CTD patients develop ILD or when evaluat… Show more

“…Anti-aminoacyl-tRNA synthetase (anti-ARS) antibodies are known to be specific to polymyositis (PM)/dermatomyositis (DM) [1-3]. Among patients with anti-ARS antibodies there is a high frequency of interstitial lung disease (ILD) (ARS-ILD) (range: 71–100%) [2].…”

“…Multidisciplinary teams should consider intrathoracic and extrathoracic disease activity. Similar to the approach taken with the diagnosis of idiopathic interstitial pneumonia [86], a clinical-radiological-pathological team approach can be used to improve the decision-making processes involved in CTD-ILD [8,12,87]. …”

“…The current definition fails to satisfy the rheumatological criteria for a definitive diagnosis of CTD [11], and fails to affirm the findings of serological tests. CTD-ILD is also known by the term “interstitial pneumonia with autoimmune features (IPAF)” [12]. It is difficult to provide a differential diagnosis of CTD-ILD with regard to drug toxicity and infection.…”
