Intestinal Surgery for Crohn’s Disease: Predictors of Recovery, Quality of Life, and Costs

Abstract: Laparoscopy was associated with a shorter postoperative length of stay; stoma creation was associated with a long and expensive postoperative hospital stay, and stricturoplasty was associated with a slower recovery of bowel function.

“…On the other hand, as expected [38], diverting colostomy creation resulted to be one of the predictors of bad BIQ scores at the multiple regression analysis. Nevertheless, similarly to what was observed in patients with Crohn's disease [39], in this series an ostomy bag did not seem to influence postoperative quality of life.…”

“…In case of entero-sigmoid fistula or large inflammatory mass, a small Pfannestiel incision (8 cm) was used instead of the transumbilical incision. 3 Vascular ligation, bowel division, and anastomosis were performed extracorporeally. Stapled anastomoses were constructed in a side-to-side fashion using an 80-mm linear stapler (GIA75; US Surgical Corporation, Norwalk, CT, USA).…”

“…The bowel was incised along its main axis on the anti-mesenteric side on a Crohn's stenosis and sutured transversally with absorbable 3-0 Vicryl (Ethicon) stitches. 3 Histology After fixation in 10% neutral-buffered formalin at room temperature for 24/48 h, the resected specimens were dehydrated and embedded in paraffin wax. Sections (3-μm thick) were then cut and standard hematoxylin/eosin stain applied was evaluated by pathologists specialized in gastrointestinal pathology and in inflammatory bowel Finally, MRE identified preoperatively only one out three neoplasms (two leiomyomas and one lymphoma) with a sensitivity of 0.33 (95% CI 0.01-0.87) and a specificity of 1.00 (95% CI 0.86-1.00).…”
