Intra-Individual Comparison of 18F-PSMA-1007 and 18F-FDG PET/CT in the Evaluation of Patients With Prostate Cancer

Abstract: Purpose18F labelled PSMA-1007 presents promising results in detecting prostate cancer (PC), while some pitfalls exists meanwhile. An intra-individual comparison of 18F-FDG and 18F-PSMA-1007 in patients with prostate cancer were aimed to be performed in the present study. Then, the pitfalls of 18F-PSMA-1007 PET/CT in imaging of patients with prostate cancer were analyzed. Methods and Material21 prostate cancer patients underwent 18F-PSMA-1007 PET/CT as well as 18F-FDG PET/CT before treatment. All positive lesion… Show more

“…Before quantification of PSMA uptake can be used as a biomarker or surrogate endpoint to identify response to treatment, and before we can design sufficiently powered response evaluation studies, a thorough understanding of parameters affecting the quantitative results is required. Uptake of FDG and PSMA differ due to pharmacodynamical differences [ 8 , 9 , 10 ]. Therefore, comparison of uptake measurements from scans with different ligands should be approached with caution.…”

“…The intensity of 18 F-FAPI uptake in organs or benign lesions was quantified in terms of the standardized uptake value (SUV) (SUVmax, and SUVmean). SUVs were acquired by measuring the volume of interest in a 1-or 2-cm spherical region drawn in the parenchyma of organs and benign lesions, according to the size of the organ and 42% threshold isocontour [24,25]. Blood pool SUV was acquired by drawing a 1-cm spherical region in the center of the descending thoracic aorta, excluding the aortic wall.…”

“…Therefore, early diagnosis and treatment are important for PCa. Between 27% and 53% of all patients undergoing radical prostatectomy or radiation therapy develop a rising prostate-specific antigen (PSA) level (PSA recurrence) (4). Importantly, patients with PSA recurrence after radical prostatectomy or primary radiation therapy have different risks of subsequent PCa-specific mortality (4).…”
