Intra-uterine versus cervical insemination of donor sperm for subfertility

Abstract: Intrauterine insemination appears to be beneficial when cervical insemination using cryopreserved donor sperm has had low pregnancy rates. This applies to spontaneous, clomiphene corrected and gonadotrophin stimulated cycles. However it may offer little benefit where high pregnancy rates have been achieved with cervical insemination. There appears to be no additional benefit from intrauterine insemination when fresh sperm is used for donor insemination.

“…The IUI results (Table 3 ) show the pregnancy rates resulting from the use of sorted sperm were comparable to rates published in the literature. Published IUI pregnancy rates are generally 10-15% per cycle [ 25 27 ], whereas the overall IUI pregnancy rate in the current study was 14.7% in a population of participants presumed to have normal fertility potential. Theoretically, the presumed normal fertility status of most of the current study’s participants could have potentially increased the IUI pregnancy rate over that reported for studies that did not employ sorted sperm.…”

“…Other evidence that supports the importance of the technique is the different pregnancy rates achieved with IUI or intracervical insemination 3 . As inappropriate techniques may lead to the use of intracervical insemination, which has lower pregnancy rates, any technique, such as filled bladder which makes IUI possible, may result in higher pregnancy rates.…”

Higher pregnancy rates and higher numbers of easy IUI in the full bladder group observed in our study is also consistent with the published reports on embryo transfer in IVF cycles.…”
