Intracranial cellular schwannomas: a clinicopathological study of 20 cases

Abstract: Aims Cellular schwannoma is a specific subtype of schwannoma, prone to misinterpretation as a malignant neoplasm. Involvement of the intracranial compartment by these tumours is extremely rare. We aim to characterise this clinicopathological subgroup. Methods and results We identified a total of 20 cellular schwannomas with predominant intracranial involvement. The mean age of the patients at the time of surgery was 37 years (range = 16–81), with a slight female predominance (1.5:1 ratio). The most common site… Show more

“…Due to the rarity and the limited reported cases, the radiologic features of intra-parenchymal and intra-ventricular schwannoma were investigated insufficiently. As observed in this study, the intra-parenchymal and intra-ventricular schwannoma commonly appeared as a solitary, rounded or oval, well-defined heterogeneous mass with calcification, cystic degeneration and delayed enhancement, It was consistent with previous studies[ 6 , 14 , 15 ]. In the present study, the intra-parenchymal and intra-ventricular schwannoma was solitary, oval and well-defined, which was potentially related to the incidence of a tumor capsule sign [ 4 ].…”

“…Intra-parenchymal and intra-ventricular schwannoma is benign in nature, which is commonly reported in young adults without gender predilection[ 6 ]. Owing to its divergent location, size and texture, the clinical symptoms could be multiplied.…”

“…Although the nomogram has achieved a good degree of discrimination, calibration, and clinical benefits, we believe that it still needs to be improved in the future; The first is to include the volume of the residual tumor as a risk factor in patients with partial tumor resection to achieve a more personalized risk prediction ( 17 ). This requires the use of standardized volume measurement software and methods to reduce measurement errors of different institutions; The second is to include more detailed pathological classification because the recurrence rate of cellular schwannomas is much higher than that of classic schwannomas ( 24 , 25 ). However, this distinction has not received extensive attention for a long time.…”

“…Based on cell morphology, schwannomas are divided into two types: Antoni A and B patterns. The Antoni A pattern often displays well-differentiated spindle cells with palisade nuclei, whereas the Antoni B pattern is characterized by bipolar and multipolar cells suspended in a loose myxoid matrix[ 19 , 20 ]. Imaging findings of schwannomas correlate with the biphasic pattern observed histopathologically.…”
