Intracranial Dural Arteriovenous Fistulas

Abstract: Dural arteriovenous fistulas (DAVFs) are abnormal communications between meningeal arteries and dural venous sinuses and/or cortical veins. Although many fistulas are benign and do not require treatment, some may carry a significant risk of bleeding or cause symptoms and warrant treatment. This review provides a review of various aspects of intracranial DAVFs including epidemiology, pathophysiology, clinical presentation, imaging characteristics, classification, natural history, and management options. “…It accounts for 10-15% of intracranial vascular malformations commonly found in the transverse sinus-sigmoid sinus area, cavernous sinus area, tentorium of the cerebellum area, and superior sagittal sinus area (1,2). DAVF usually occurs in people aged 50-70 years and is slightly more common in men than in women (3). The pathophysiology of DAVF is an obstruction caused by inflammation, thrombosis, and stenosis, which leads to congestion and venous hypertension.…”

“…1 Dural arteriovenous fistulas (DAVFs) are abnormal shingles between the arterial and venous systems within the dural membrane, accounting for about 10% of intracranial vascular malformations, and about 20%–33% of DAVFs present as intracranial hemorrhage. 2 Although most DAVFs patients do not experience significant discomfort, some patients may experience significant complications, 3 namely manifestations of venous hypertension, including varicose veins, pain and swelling, pulsatile tinnitus, etc. In severe cases, intracranial hemorrhage or edema, seizures, and other symptoms may occur.…”
