Intracranial germ-cell tumours — treatment results and residuals

Abstract: After surgical removal and radiation therapy long-term survival of intracranial germinomas amounts to 88%. Despite craniospinal axis radiation severe residuals are rare and a good quality of life is common. In malignant teratomas treatment regimens including chemotherapy are much less successful.

“…However, due to diffuse infiltration of the peritumorous venes in the 4th case with a malignant teratoma, only subtotal removal was achieved. In our overall series of 17 children with intracranial germ cell tumors, severe postoperative morbidity was observed neither after subtotal (n = 7) nor after total (n = 10) tumor removal [16].…”

“…Since intracranial marker-positive nongerminomatous germ cell tumors are a rare entity, and even in large series of intracranial germ cell tumors constitute only a few cases [5,15,16], experience with their treatment is limited in any center. Therefore, the creation of a central register for this kind of tumors seems to be desirable.…”

“…Intracranial germ cell tumours are rare brain tumours and occur in 3–11% of children and adolescents with malignant CNS tumours, with a higher incidence in eastern countries [2, 7, 8]. Germinomas account for about 65% and teratomas for 20–30% of intracranial germ cell tumours, while choriocarcinomas, yolk sac tumours, or embryonal carcinomas are only found in up to 1% of the patients [2, 8].…”
