Intracranial subdural hematoma following lumbar myelography

Abstract: Persistent headaches after lumbar puncture or myelography can be due to intracranial hematoma. This possibility should be evaluated by computerized tomography, keeping in mind the difficulty in the diagnosis of the isodense subdural hematoma.

“…Lomber ponksiyon sonrası PDPB, olguların yaklaşık %40'ında görülen ciddi bir klinik bulgudur. [26] Bizim çalışmamızda da, hastalar tarafından en sık belirtilen GNS bulgusu, başağrısı idi (%15.4). Çalışmamızda, işlem sonrası ortaya çıkan başağrısı sorgulanmıştır.…”

“…Headache is the most frequent complication after lumbar puncture, occurring in`40% of cases. 6 Such headaches are thought to be caused by excessive (`250 ml day ±1 ) 7 leakage of cerebrospinal¯uid (CSF) through the dural puncture, causing caudal displacement of intracranial structures. 8 This displacement then stretches the intracranial pain-sensitive dura, sinuses and blood vessels, causing pain.…”

“…This report illustrates that cerebrospinal fl uid leakage after a dural tear during spinal surgery may cause intracranial hemorrhage. A previous head trauma could be a risk factor for such I ntracranial hemorrhage after cerebrospinal fl uid (CSF) leakage after medical procedures at the spinal level, such as dural lumbar puncture, 1-3 spinal anesthesia, 4-5 myelography, 6 and spinal surgery, 7-28 is a poorly recognized complication. We present a rare case of both supratentorial subdural hemorrhage (SDH) and cerebellar hemorrhage (CH) after spinal surgery.…”
