INTRODUCTION. How to reduce the fatality of coronavirus disease (COVID-19) is still challenging. A proper nutritional support has been always a matter of attention in critically ill patients. MATERIAL AND METHODS. We assessed COVID-19 patients who had received intralipid infusion due to medical indications and compared them with those who did not receive it regarding fatality rate and prognosis. As a part of a data mining project using data of observational cohort of COVID-19 patients hospitalized in the educational centers of Iran University of Medical Sciences, Tehran, Iran, an inferential case series was performed. A total of 19 patients with SARS-CoV-2 infection were selected from the cohort. Briefly, 13 patients survived and 6 patients died, and 12 patients were admitted in intensive care unit (ICU). All dead cases were ICU admitted. The association of intralipid infusion and survival rate was examined using Fisher exact test. No association was observed between intralipid infusion and survival. CONCLUSIONS. No significant protecting effect was observed for patients who received intralipid for medical indications. Since intralipid was administered according to medical indications, surviving of all the non-ICU admitted patients despite having underlying diseases was remarkable. Despite the fact, due to several bias factors that could not be controlled in such a retrospective study, the results might be accidental. We suggest to assess such an effect retrospectively in other centers as well.

In the modern world Total Quality Management (TQM) respecting the past ensures the triumph of the future and Artificial Intelligence. Fulfilling the absence in the universal medical journal of multi-organ supportive therapy (MOST) replacement models of the central nervous system, immune and energy system functions, have been completed by describing extracorporeal and intracorporeal prosthetics of immune and energy system functions. Prosthetics, assistive support, and replacement of nervous system function contributed to the foundation of Artificial Intelligence (AI) and the creation of the robot as an intellect-technocrat combination. Using the Da Vinci robot with four arms, surgeons in Spain performed a lung transplant [3], relevant and timely also in the complications of COVID-19 SARS-CoV-2. As a result, the robot reduced the discrepancy between the predominance of the human and technocratic intellectual factors in the creation of the product useful to humanity. At the foundation of AI is TQM recognized by the scientific community of the modern world and in the context of alternative medicine models as a promoter of science. Thanks to the US development over 40 years of transgenic pig organ transplantation, as a model conceived in Moldova (1984), as an alternative to human donor organs. The prized TQM medicine is successfully assimilated by competitive marketing, in which AI is already included, with an essential social financial role. IA was supported by Antonio Guterres, Secretary General of the United Nations, Ursula von der Leyen, President of the European Commission. Thus AI has been considered at the level of the United Nations, the European Union, Japan, China and others. The head of the European Commission, Ursula von der Leyen from Great Britain at the First AI Safety Summit in the world, named 4 conditions for ensuring safety in the use of AI. 1) A thriving and independent scientific community equipped to evaluate AI systems, with funding and access to the best supercomputers; 2) Development of internationally recognized procedure and standards for AI safety testing. 3) A standard procedure whereby every significant incident caused by errors or misuse of AI is reported and accounted for. 4) Establishment of a reliable international notification system.

According to the World Health Organization (WHO), coronavirus infection (COVID-19, (COronaVIrus Disease 2019) is an infectious disease caused by the SARS-CoV-2 virus [1]. SARS-CoV-2 virus, Severe Acute Respiratory Syndrome-related coronavirus 2. Initially considered pulmonary - respiratory pathology, then it turned out to be poly-systemic, with multi-inflammation of many other organs, and central nervous system. Multisystem inflammatory syndrome in adults (MIS-A) [2], SARS-CoV-2 liver infection [3]. In pediatrics Multisystem inflammatory syndrome in children (MIS-C) or PMIS-TS (Pediatric MIS temporarily associated with SARS-CoV-2 infection) [4,5], SARS-COV-2/ COVID-19 in children induce Kawasaki-Like Disease [6]. Neurological manifestations [7], recurrence of COVID-19 infection with meningitis without pulmonary involvement [8]. Brain damage in COVID is primarily neurological, as a material signaling pathway of inflammatory COVID-infection of cerebral vessels. This is important, from the release of these patients, from additional stress, suffering, stigmatization in society, their isolation, intimidation, coercion (bullying) with violation and restriction of their rights. Since, concomitantly with inflammatory lesions of many organs, cognitive deficits are a component of the post-acute consequences of COVID-19 (PASC), where the role of the kynurenine pathway is significant with temporary cognitive impairment [9]. The main pathway for the breakdown of tryptophan is an amino acid, without which the synthesis of serotonin is impossible, which controls the cycles of sleep and wakefulness, which is the precursor of the sleep hormone melatonin In patients with COVID-19, an imbalance of T-helpers 1 and 2 leads to a cytokine storm that can contribute to myocardial damage [10]. MIS is conditioned caused by immunocompromising (IC) CHAOS - [C]ardiovascular Compromise: shock; [H]omeostasis; [A]poptosis; [O]rgan Dysfunction; [S]uppression of the Immune System with the development of Systemic inflammatory response syndrome (SIRS) and Multiple Organ Dysfunction Syndrome (MODS) [11,12]. With pronounced multiple and other symptoms: Electro - Ion Membrane Distress Syndrome (Syndrome Maria & Irina Vasilieva) [13,14] with membrane manifestations of electrical storm or electromembrane paralysis; Capillary Leak Syndrome; Microcirculatory-Mitochondrial Distress Syndrom [15,16]. Also described: Overlap mechanisms of transient global amnesia and COVID-19 infection [17]; Hypokalemia-stroke mimic [18]; Differential diagnosis of cacosmia and dysgeusia in COVID-19 pandemic [19].
