Intraluminal Duodenal Diverticulum Causing Acute Pancreatitis: CT Scan Diagnosis and Review of the Literature

Abstract: Background: Intraluminal duodenal diverticulum is a rare congenital anomaly. First described by Boyd in 1845, no more than 100 cases have been reported up to now: only 17 are associated with acute pancreatitis. Methods: A new case of intraluminal duodenal diverticulum with acute pancreatitis is reported and the literature about this association reviewed. Results: The diagnosis was made by helical CT scan. The pathogenesis of pancreatitis was possibly due to a pure duodenal content reflux through the papilla of… Show more

“…The MRI findings of this entity have not been described previously. Our findings on MRI correlate well with previously described CT findings, which included detection of an air-fluid level (10,11). On MRI, both HASTE and True-FISP techniques demonstrated air-fluid levels.…”

“…Sometimes diverticulum becomes symptomatic with epigastric pain, nausea and vomiting after meal. In 1% to 5%, complications occur such as hemorragic ulcer (25%), acute pancreatitis (20%) by stretching the major papillar with Oddi's sphincter dysfunction, and proximal digestive obstruction [3]. In the present case, CT scan nicely demonstrates the mechanism of pancreatitis.…”

“…1,2 The reported incidence of pancreatitis associated with IDD is <20% and it mainly affects young adults. 7 The pathogenesis of pancreatitis in patients with IDD is not clear. The most widely accepted mechanism is the reflux of the duodenal content through the papilla of Vater.…”
