Intranasal sodium hyaluronate on the nasal cytology of patients with allergic and nonallergic rhinitis

Abstract: The addition of sodium hyaluronate to intranasal corticosteroid and systemic antihistamine reduced the neutrophil count seen on nasal cytology in patients with allergic and nonallergic rhinitis and improved several clinical and endoscopic parameters while being well tolerated. These data provide encouraging evidence of the efficacy of sodium hyaluronate in the treatment of this common disease.

“…HYA is an active component of two inhalation products. Yabro ® (Ibsa Farmaceutici, IT) is a high viscosity hyaluronate solution for nebulization (MW 800-1000 kDa, 0.3% w/v) to reduce bronchial reactivity induced by inhalation of allergens/pollutants or by physical effort (Gelardi et al, 2013). In this formulation, HYA improves palatability and reduces potential side effects of nebuliser solutions such as irritation and cough.…”

“…Samples were placed on a glass slide, fixed by air drying and stained according to the May-Grunwald Giemsa (IR MEDICAL, Lugo, RA, Italy) method and then observed by optical microscope (13). The nasal cytological assessment, performed in blind condition by two investigators who were unaware ofthe assigned treatment, evaluated the presence and the number of neutrophils, eosinophils, mast cells, bacteria and fungi according to a 0-4 score (0 = absent, 1 = rare, 2 = some, 3 = easily visible, 4 = many) (13). Ciliary motility was assessed on fresh samples by contrast-phase microscope according to a 0-3 score (0 = absent, 1 = < 5',2 = 2: 5' and:::: 10',3 = > 10'), where a higher value corresponds to a better motility (14).…”

“…Hence, exogenous HMW HA represents a promising therapeutic agent for the treatment of nasal and lung diseases that involve inflammation, oxidative stress and epithelial remodeling, such as allergic and non-allergic rhinitis, asthma, chronic obstructive pulmonary disease and cystic fibrosis [143,233,[235][236][237][238]. Examples of marketed formulations containing HA to treat respiratory diseases are Ialoclean® (Farma-Derma) a nasal spray to treat nasal dryness and rhinitis, and to promote nasal wound healing-, Hyaneb® (Chiesi Farmaceutici) -a hypertonic saline …”

“…On the other hand, crosslinking is normally intended to improve the mechanical, rheological and swelling properties of HA, and reduce its degradation rate, in order to develop derivatives with a longer residence time in the site of application and greater release properties [58,138,139]. A recent trend is to conjugate and crosslink HA chains using bioactive molecules in order to develop derivatives with improved and customized activities [58] for a variety of applications in medicine, aesthetic and bioengineering -including cell and molecule delivery, tissue engineering, and development of scaffolds [46,56,58,[140][141][142][143][144]. A number of synthetic approaches have been developed to produce conjugated or crosslinked hyaluronan [136].…”
