Intraoperative Convulsions in a Child with Arthrogryposis

Abstract: A case of intraoperative convulsions occurring in a child with arthrogryposis multiplex congenita is presented. Arthrogryposis and the anaesthetic management of children with this condition is discussed. Factors which may have contributed to the convulsions are considered.

“…Froster‐Iskenius reported suspected MH in two siblings affected by arthrogryposis (6). There have also been reports of an intraoperative hypermetabolic response to anesthesia in AMC, which is described as a rise in body temperature that responds to active cooling (15–18). No definitive cause or underlying defect responsible for the hyperthermia has been identified; rather it has been proposed that this a yet unexplained exaggerated metabolic response to anesthesia or surgical stress that is distinct from MH and does not require therapy other than control of body temperature, which has occasionally entailed surface cooling (15).…”

“…The issue of association of arthrogryposis multiplex congenita with malignant hyperthermia has been widely debated [14][15][16][17][18] . Recent reports do not support this association.…”
