Intrapartum assessment of fetal well-being: any role for a fetal admission test?

Abstract: The fetal admission test is useful in predicting the absence of intrapartum fetal distress irrespective of the criterion used for evaluation. Redefined reactivity appears to be most predictive of intrapartum fetal distress.

“…They found the fetal admission test to be useful in low-risk populations. 12 Blix et al reported lower sensitivity (15%) and PPV (16%) of labor AT in high-and low-risk populations compared to this study. They also indicated that there were no differences in women with equivocal labor AT and those with an ominous one delivering a baby with fetal distress.…”

“…If admission CTG were suspicious, other tests would be more reliable predictors. 11 Elimian et al (2003) stated that patients in the non-reactive group were six times as likely to have fetal distress, and that fetuses with distress were more likely to have a non-reactive AT. They found the fetal admission test to be useful in low-risk populations.…”

“…A systematic review has previously assessed the prognostic values of the admission CTG . Eleven observational studies including 5831 women were included in the review . A new literature search was performed in June 2012, and one study that met the inclusion criteria was identified .…”

“…A study by Kulkarni and Shroti also showed a progressive rise in operative deliveries for fetal distress from 5.17% in the reactive group to 28.5% in the ominous group [15]. Other studies also established the association between a high Cesarean Section rate and an abnormal cardiotocography [16] [17]. It was noticed that rate of Cesarean Section increased when cardiotocography were performed for low risk pregnancies.…”
