Intraperitoneal catheter drainage of lymphocele: an outpatient procedure

Abstract: Summary Laparoscopic marsupialization of lymphocele carries 13% recurrence rate, 6% injury to other organs, 12% omentoplasty, 6% open conversion and 1.8 average hospital days. A novel, simplified technique of intraperitoneal catheter drainage of lymphocele is described. Under ultrasound guidance and using the Seldinger technique, a 13F Hickman catheter was introduced into the lymphocele and connected subcutaneously to a small peritoneal window performed 5 cm apart. During the last 8 years the procedure was per… Show more

“…Lymphocele often occurs within 3 ∼ 8 weeks or 1 year occasionally after surgeries [ 38 ]. Because of self-limiting, most of postoperative lymphocele is usually asymptomatic, undiagnosed and self-healing without any treatments [ 38 , 39 ]. Only 4% – 7% of postoperative lymphocele is symptomatic [ 16 ] due to self-absorption disorder.…”

“…Laparoscopic lymphocele fenestration has yielded very promising results. In a study, with this method, under local anesthesia and guided by ultrasonography, intraperitoneal catheter drainage was performed with a 92.8% success rate (19).…”

“…It should be noted, however, that in the case of inferolateral or posterolateral lymphoceles, laparoscopy is hazardous because of a risk of vascular damage by trochar (16). Furthermore, one study stated that peritoneal marsupialization of a deep lymphocele in an obese patient led to a higher recurrence rate because of the difficulty in suturing the peritoneum to the lymphocele wall and draining the lymphocele completely (19). For the prevention of lymphocele, we suggest a careful ligation of the lymph ducts in the donor's kidney.…”

“…Die Inzidenz wird in der Literatur sehr unterschiedlich zwischen 0,6 und 26,0% angegeben [2,3,4]. Häufig sind Lymphozelen klein und asymptomatisch und bedürfen keiner chirurgischen Intervention [5]. Ursachen für die Entstehung von Lymphozelen sind u. a. die Sekretion aus Lymphgefäßen des Transplantathilus sowie aus den begleitenden Lymphbahnen der Empfängergefäße [6].…”
