Intravasation of barium sulphate at barium enema examination

Abstract: We report a case of venous intravasation of barium sulphate occurring during a routine barium enema examination for investigation of rectal bleeding. The patient suffered a cardiopulmonary arrest, but made a full recovery after organ support in intensive care. Review of radiographs from the examination showed intravasated barium in pelvic vessels. We review the literature on this rare, but serious, complication of barium enema examination and suggest measures by which intravasation can be prevented.

“…Barium enema is usually considered a safe investigation, and the most common serious complication is bowel perforation, which occurs in 0.004–0.04% of cases. 1 The most fearsome complication, although rare, is barium intravasation, which leads to massive pulmonary embolism, and sudden death. The mortality rate of this complication has been reported to be as high as 67%.…”

“…Barium intravasation via lower rectum or vagina usually enters the systemic circulation through the internal iliac veins and then into the lungs where it causes occlusion to the pulmonary circulation. 1 It may also enter the portal circulation via upper rectum and colonic vessels. 1 In this patient, it is evident that she has intravasation into the vagina.…”

“…Ainda podemos considerar um sexto tipo, o intravasamento de sulfato de bário para as veias de drenagem do segmento colorretal acometido. (9,20) A infiltração anorretal é uma forma de perfuração incompleta que ocorre quando o sulfato de bário é forçado através da descontinuidade da mucosa retal, formando granulomas de bário na submucosa do reto e anoderma. (7) As manifestações clínicas da perfuração podem ser diferentes de acordo com o tipo de lesão.…”

“…Although hepatic deposits may persist for years, the liver prevents systemic migration of barium. 4,5 Intravasation of barium sulfate into the systemic circulation carries a worse prognosis, and at least seven fatalities from barium-induced massive pulmonary embolism have been reported. 6 We report a case of barium sulfate injection directly into the superior vena cava during a UGIS in which the patient's central venous line (CVL) was mistaken for her gastrostomy tube.…”
