Intravenous Benzodiazepines as Anaesthetic Agents: Pharmacokinetics and Clinical Consequences

Abstract: Despite extensive and numerous pharmacokinetic studies on benzodiazepines, the published pharmacokinetic data do not adequately explain the clinical differences found between different benzodiazepine derivatives after intravenous administration. Especially, correlations between initial drug responses and distributional changes of the benzodiazepines are limited. However, during the elimination phase some relationships exist between the kinetic and dynamic phenomena. Age, sex, diseases and concomitantly given d… Show more

“…Total distribution volume and clearance values of midazolam agreed with previous data (Smith et a1 1981). As anticipated, apparent elimination half-life values of midazolam were slightly longer than those reported during single dose studies (Kanto & Klotz 1982;Smith et a1 1981). There was no difference in the pharmacokinetic parameters calculated using the serum levels determined by RRA or GLC (Aaltonen et a1 1985).…”

“…It has the characteristic properties of benzodiazepines, i.e. sleep inducing, sedative, anxiolytic, anticonvulsant and muscle relaxant effects coupled with a short elimination half-life of about 2 h (Kanto & Klotz 1982;Smith et a1 1981) and the watersolubility of its salts (maleinate).…”

“…The individual response varied severalfold. This clinical problem has also been observed with other benzodiazepines and might complicate their use in anaesthesiology (for review see Kanto and Klotz, 1982). Since plasma concentrations and free fractions of midazolam were very similar in our subjects, the different response might be due to variability in penetration into cerebrospinal fluid (CSF) and/or to the number/affinity of the benzodiazepine receptors in the brain.…”

“…The amnesic effect of preanesthetic medication may not be desirable in every patient because it is often associated with loss of self-control that may be alarming (22).…”
