Intravesical bacillus Calmette‐Guèrin therapy: experience with a reduced dwell‐time in patients with pronounced side‐effects

Abstract: patients self-reported side-effects after each instillation in a questionnaire. RESULTSAfter reducing the BCG dwell-time, fever, chills, dysuria and the overall time-torecovery were significantly reduced but frequency and haematuria were not influenced. Patients with carcinoma in situ had significantly less dysuria than patients with papillary tumours. There was no difference in the treatment results between patients who had a normal dwell-time and a reduced dwell-time, determined at the first and second follo… Show more

“…The Tice and Connaught strains have similar side effects (42% vs. 28%, P = 0.09) [25] and no difference in intolerability for the Tokyo and Connaught strains (8.1% vs. 9.8%, P = 0.70) was shown [26]. Andius et al reduced the instillation dwell time to less than 30 min [27]. Decreased fever, chills and dysuria were found while frequency and haematuria were unchanged.…”

“…Reduction of dwell time has been reported as a possibility for improving therapy and as an alternative to dose reduction in patients with severe side effects [16] but no prospective study has compared BCG dwell time as a variable. Our model indicates that increased dwell time might also influence treatment outcome ( Figure 2C ).…”

“…However, the duration can be reduced as an alternative to dose reduction in patients with significant side effects. [19] A standard induction course consists of six weekly instillations. Maintenance is typically given as three weekly instillations at 3 and 6 months and then every 6 months for up to 3 years.…”
