Adverse childhood experiences (ACEs) and trauma have been linked to decreased psychosocial and physiological health functioning. While various individual and community-level interventions to address ACEs have been reported, one novel approach that has not been explored in detail is a community-engaged causal loop diagramming project, or systems mapping project (SMP), in which diverse stakeholders work together to document the forces that are creating the outcomes and patterns within the community. To better document and understand the impact of participation in an SMP, we conducted in-depth, qualitative interviews with 16 stakeholders who were involved in a systems-mapping process facilitated by a local nonprofit in Eastern North Carolina. We used an iterative, content analysis coding process to generate and analyze themes from these interviews. Three major themes emerged: 1) Recognition and understanding of own trauma, 2) Trauma as both a community issue and an individual issue, and 3) Systems-mapping as a conceptual tool with practical benefits. All participants strongly recommended the systems-mapping approach to other communities and believed that it is a valuable tool for empowerment and provided several considerations for future organizers of similar systems-mapping projects. Our findings suggest that systems mapping is a feasible, transferable, and promising modality for understanding and addressing ACEs at the individual, interpersonal, and community-levels, as well as for putting community voices at the forefront of efforts to address ACEs.

Community engagement (CE) is an important component of public health research and program implementation, especially in low- and middle-income countries. More recently, CE activities have been utilized to develop partnerships in research and program implementation processes, and advocate for policy recommendations with the aim to improve acceptance and reduce disparities of public health research activities and benefits in the involved communities. Utilizing the tacit knowledge gained from the Global Polio Eradication Initiative, this paper highlights the contributors and challenges to the implementation of the GPEI program’s community engagement initiatives from an implementers’ perspective. The study took a mixed methods approach to analyze data collected from the Synthesis and Translation of Research and Innovations from Polio Eradication (STRIPE) project, which conducted an online survey and hosted key informant interviews with individuals who had been engaged with the GPEI program from 1988 onwards for at least 12 or more continuous months. An analysis of data limited to individuals (32%, N = 3659) who were primarily involved in CE activities revealed that around 24% were front-line healthcare workers, 21% were supervisors and 8% were surveillance officers. CE activities mainly focused on building trust within the communities, addressing misinformation, myths and fears around vaccinations, mobilization to reach high-risk or hard to reach populations, as well as building ownership and buy in from the communities. The strength of the implemental process of a program (38.7%) was among the key drivers of success, coupled with personal beliefs and characteristics of the implementers (25.3%). Social, political, and financial forces received mixed opinions as to their importance, depending on the stage of execution and readiness of the communities to accept the programs. Lessons learnt from the GPEI program provide tried and tested best practices and evidence for strategies that would work in diverse backgrounds with some customization to suit the needs of the situation.

Objective: The quality of patient-provider relationships (PPR) is directly related to delivering patient-centered care and improved healthcare outcomes (e.g., adherence to medical treatments) and differs across types of patients to the point of health disparities. PPRs are further complicated when factoring in multiple chronic health conditions, language barriers, and limited time. This qualitative longitudinal study explored the perceived acquiring of high-quality PPR among older foreign-born Latinos over time. Methods: We recruited 13 patients with multi-morbidities from nine Program of All-Inclusive Care for the Elderly (PACE) centers. Researchers conducted three rounds of in-depth interviews in Spanish (N=39 interviews) over 13 months. The first interviews were conducted face-to-face and lasted one hour on average. The second and third were conducted over the phone and ranged from 60 to 90 minutes. Data were analyzed using line-by-line in vivo coding, identifying 倡导者。各层面彼此依赖。第一个层面(好医生)是最表面的，因为它侧重于医生的技术专长。第二个层面(可信赖的医生)随时间推移表现出值得信赖的特征。第三个层面(倡导者)体现了可信赖的人，再加上额外的倡导维度。结论：独特的研究特征允许探究PPR。研究发现阐明了一系列有助于发展稳固的PPR的因素(例如，专业知识、信任、倡导) 。为加强PPR，应考虑相关策略来建立信任和倡导患者需求。关键词：封闭系统，拉丁裔，老龄化，信任，定性研究，半结构化访谈，纵向研究，护理的连续性
