Intussusception of the small bowel discovered incidentally by computed tomography

Abstract: Three cases of asymptomatic intussusception in adults are reported, discovered incidentally during a CT scan of the abdomen. It is being increasingly observed in adults, possibly aided by the reformatting capabilities of the new multislice CT scanners. It has been documented in children where it is considered to be transient and of no clinical significance.

“…Given this increased detection of intussusception, even in asymptomatic patients, and given the recognition that some may be transient, there is an ongoing controversy regarding the optimal management of these cases. The general consensus is that proximal small-bowel intussusceptions measuring less than 3.5 cm in length and without obstruction or an obvious lead point are likely to be selflimiting and of no clinical significance [30], and larger sections should be monitored conservatively [31], only resorting to surgery if complications arise.…”

“…Incidental intussusception was defined as intussusception detected on CT in patients with no symptoms attributable to the intussusception. 16 They are often transient and asymptomatic and usually do not appear on subsequent imaging. 17 Idiopathic (primary) intussusception was defined as an intussusception for which no etiology or lead point could be identified by imaging, exploration, and/or pathologic evaluation regardless of symptoms.…”

“…Dysrhythmic peristaltic activity has been proposed as the cause of intussusception in horses and people. Asymptomatic small bowel intussusceptions occur in humans and dogs and can resolve spontaneously (up to 20% in people and 8% in dogs), suggesting that this process might be underreported . Sonographic signs that have been associated with transient, nonclinical small intestinal intussusceptions are the absence of identifiable intestinal lesions, normal wall thickness, length of less than 3.5 cm, normal undilated proximal bowel, normal vascularity on color Doppler, respected layering, and the intussusception being compressible .…”

“…The authors had different levels of experience, ranging from 6 months to 30 years of ultrasound experience. Intussusceptions were identified in cross‐section if intestine with a classically described target‐like or doughnut appearance was observed, and in longitudinal section when the classically described sandwich sign was observed . One of the authors (MA) measured wall thickness of the intussusceptum and the intussuscipiens for each foal.…”
