Invasive zygomycosis in India: experience in a tertiary care hospital

Abstract: Zygomycosis is a threat in uncontrolled diabetes. New risk factors such as renal failure and chronic liver disease require attention. A elegans is an emerging agent in India. The need for surgical debridement in addition to medical treatment is emphasised. GCS is an independent marker of prognosis in cases of invasive zygomycosis.

“…Three case series on mucormycosis have been reported from our tertiary-care centre: 129 cases over 10 years (1990)(1991)(1992)(1993)(1994)(1995)(1996)(1997)(1998)(1999), 178 cases during the subsequent five years (2000)(2001)(2002)(2003)(2004) and then 75 cases in an 18 month period during (Chakrabarti et al, 2001, 2009. Rhizopus oryzae and Apophysomyces elegans (sensu lato) were the predominant isolates in these series (Chakrabarti et al, 2001(Chakrabarti et al, , 2006(Chakrabarti et al, , 2009.…”

“…Aspergilloma, a localized infection, has been shown to consist of highly agglutinated hyphae encased in an extracellular matrix (Loussert et al, 2010;Mowat et al, 2009). A similar exopolysaccharide matrix is also produced at the surface of fungal hyphae during invasive pulmonary aspergillosis (Loussert et al, 2010 and fungal-ball formation (Chakrabarti et al, 2001(Chakrabarti et al, , 2006(Chakrabarti et al, , 2009Goodnight et al, 1993;Kirkpatrick et al, 1979;Lahiri et al, 2001;Robey et al, 2009). Mucormycosis is categorized into rhino-orbito-cerebral (ROC), pulmonary, gastrointestinal, cutaneous and disseminated types depending upon the clinical presentation and the anatomical sites involved (Chakrabarti et al, 2001(Chakrabarti et al, , 2006(Chakrabarti et al, , 2009.…”

“…An extensive review of literature by Mignogna et [12][13][14] The higher prevalence of mucormycosis in India can be related to climatic and socioeconomic conditions and also to large diabetic population. The fungi has an affinity for blood vessels which they clog hampering the blood supply to the recipient bone that secondarily gets necrosed.…”

“…Deferoxamine binds to the host iron and is presented to the microorganism as siderophores, inducing sporulation and development of zygomycosis (10,15). Pulmonary zygomycosis is seen in patients with renal failure (16). Chronic renal failure was present in 3 of the cases: 2 patients with the rhino-orbitocerebral form and 1 patient with the disseminated form, all of whom died.…”
