Investigation and management of serum sodium after subarachnoid haemorrhage (SaSH): a survey of practice in the United Kingdom and Republic of Ireland

Abstract: Background: Hyponatraemia is a common complication of aneurysmal subarachnoid haemorrhage (SAH). We aimed to determine current neurosurgical practice for the identification, investigation and management of hyponatraemia after SAH. Methods: An online questionnaire was completed by UK and Irish neurosurgical trainees and consultant collaborators in the Sodium after Subarachnoid Haemorrhage (SaSH) audit. Results: Between August 2019 and June 2020, 43 responses were received from 31 of 32 UK and Ireland adult neur… Show more

“…In this prospective, multicentre, study of patients with SAH, we found that guidance for the screening and investigation of hyponatraemia was adhered to infrequently 10 11. This contrasts findings of a linked survey in which all NSUs subjectively reported measurement of serum sodium daily or on alternate days, and the majority reported the routine paired measurement of serum and urine osmolalities as well as urinary sodium to investigate hyponatraemia:7 in practice, they were measured in just 18% of cases.…”

“…There is no strong evidence to support common treatments for hyponatraemia. As such, the clinical significance of hyponatraemia after SAH is debated 4 7 8…”

“…The usage rates of treatments of hyponatremia by the NINDS StrokeNet sites contrast those reported in the survey of 32 neurosurgical units in the United Kingdom and Ireland (15). In the latter survey, treatments used for hyponatremia were enteral salt supplementation (N=24, 75%), fluid restriction (N=18, 56.2%), hypertonic saline (N=17, 53.1%), and fludrocortisone (N=7, 21.9%).…”

“…Intracerebral fluid shifts due to these abnormalities have the potential to lead to poor outcomes (11, 12); however, studies examining the relationship between dysnatremia and outcome have reported conflicting results (4). Therefore, there is uncertainty in how best to manage changes in plasma sodium concentrations in patients with aSAH (13).…”
