Ipsilateral hemiparesis after putaminal hemorrhage due to uncrossed pyramidal tract

Abstract: The pyramidal tract and the dorsal column-medial lemniscus pathway did not cross in the medulla in this patient. In view of the presence of the abnormal shape in the medulla and congenital scoliosis, a congenital factor might be responsible for the uncrossed pyramidal tract and dorsal column-medial lemniscus in this patient.

“…Terakawa et al [18] described this finding of congenitally uncrossed pyramidal tracts, with ipsilateral projection from the primary motor cortex in a man with a history of congenital scoliosis. The patient developed sudden onset right hemiparesis and was found to have a right internal capsule and basal ganglia hemorrhage on head CT scan.…”

“…Sensory-evoked potentials showed cortical response ipsilateral to the side of stimulation. The presence of the abnormal shape of the medulla in addition to the congenital scoliosis both suggest that congenital anomaly might be responsible for uncrossed pyramidal tracts as well [18]. …”

“…gaze were reported to have right hemipareses after intracranial hemorrhages on the right [10,11], but these individuals were not genotyped. More recently, a single genotyped individual had diffusion tensor MRI that vividly illustrated ipsilateral anatomic projection of the ascending medial lemniscal and descending corticospinal pathways, but this individual had no clinical abnormalities beyond HGPPS.…”

“…After exclusion of non-relevant articles, they left with total of 8 cases from the literature and 2 cases of their own.All cases were hospital based and reported retrospectively. Table.1.-summary of case reports and series if hemiparesis with ipsilateral hemiplegia/hemiparesis with unilateral stroke (Alurkar et al [1] , Terakawa et al [6] , Hosokawa et al [2] , Song et al [4] , Ago et al [5] , Ng et al [9] , Kang and Choi [7]) Table shows total of 10 patients, 7men, 2 women and 1 who s gender was not recorded. 8 Ischemic supra-tensorial Stroke and 2 Hemorrhagic Strokes (ICH)…”
