Iris mattress suture: a technique for sectoral iris defect repair

Abstract: Achieving a cosmetic and functional outcome from iris defect repair is a surgical challenge. We describe an adaptation of techniques to address a case of 2.5 clock hours of sectoral iris tissue defect. Our method combines Siepser's modified closed-chamber sliding knot technique with the placement of a double-armed iris mattress suture to approximate iris tissue to the scleral wall and thereby create a pseudo-iris root. This technique reduces glare and achieves a cosmetic outcome for the patient.

“…This study was carried out to correct the iris defects by using SFTP, and the results were compared among congenital and traumatic iris defects. Iris defects in the past have been repaired using various techniques of pupilloplasty such as Mc Cannel suture,[ 6 7 8 ] modified Siepser knot,[ 9 10 ] iris cerclage,[ 11 12 ] iris mattress suture,[ 13 ] sewing machine technique,[ 14 ] and iris cautery. “…Numerous iris suturing techniques and pupil reconstruction procedures have been practiced in ophthalmology for more than four decades. [1234567891011] The indications varied widely including congenital iris deformities, traumatic iris tear or dialysis, and pupil abnormalities due to surgery. Other than regular pupilloplasty procedures, few other indications have been introduced in recent years.…”

“…There are several iris repair techniques reported in the literature which are performed for iris coloboma, iris trauma, iridodialysis, and iatrogenic iris tear. [1234567891011] However, there are no reports concerning the assessment of post surgical configuration changes in such eyes. Moreover, the intraocular polypropylene or prolene suture, which is commonly used in iris repair, has also not been widely studied after pupilloplasty.…”
