“…Other reports described increased risk for iris retraction and poor surgical prognosis in cases of retinal detachment with associated ciliochoroidal detachment [ 3 , 4 ]. In 1984, Campbell [ 5 ] was the first to name the condition IRS and proposed the most commonly accepted mechanism for the condition, in which seclusion of the pupil and egress of subretinal fluid are the drivers of iris retraction. However, debate remains regarding the mechanism of fluid egress that leads to hypotony, as Geyer et al [ 6 ] reported 2 cases of IRS associated with nonrhegmatogenous retinal detachment.…”

“…The definitive clinical findings of IRS included retraction of the peripheral iris, hypotony and rhegmatogenous retinal detachment (Hirschberg 1929;Graham 1958;Lindner 1936;Campbell 1984;Morinelli et al 1996). The pathogenesis of IRS remains controversial: vitreous retraction with zonular relaxation (Hirschberg 1929;Graham 1958) and proliferative vitreous retinopathy (Gottlieb 1972;Jarrett 1981) were suggested as being implicated.…”

“…The pathogenesis of IRS remains controversial: vitreous retraction with zonular relaxation (Hirschberg 1929;Graham 1958) and proliferative vitreous retinopathy (Gottlieb 1972;Jarrett 1981) were suggested as being implicated. Campbell (1984) proposed a hydrodynamic theory to explain the findings whereby the retinal tears present in the rhegmatogenous retinal detachment expose the fluid of the eye to the choroidal and pigment epithelium pump. Thus, the rate of absorption of the fluid from the subretinal space exceeds the production of aqueous fluid, often suppressed by inflammation, resulting in the pulling of the iris into a retraction configuration.…”
