Is brown adipose tissue a new target for obesity therapy?

Today there is a worldwide trend of population aging, in addition, the proportion of older people with obesity is increasing. In order to adequately manage these patients in the outpatient setting, it is critical to understand the dynamic relationship between body weight, chronic disease prevalence, development of functional disability, life expectancy, and health care costs. While the obesity epidemic has affected people of all age groups, empirical knowledge about the management of obese older people remains largely scarce. Currently, there is no single therapeutic approach to this problem. Weight loss should be treated with caution in people over 60 years of age. The risk of sarcopenia, malnutrition, bone loss, increased risk of falls and injury must be considered, which can lead to an increase in the number of people with disabilities and the associated costs of medical and social care. The analytical review focuses on the relationship between two of the greatest epidemiological trends: aging and obesity. This paper highlights the features of the pathophysiology of obesity in the elderly, the phenomenon of the «obesity paradox». Also from the perspective of evidence-based medicine approaches to the treatment of obesity in the elderly, including surgical interventions aimed at reducing body weight, are analyzed.

Currently, in Russia and in the world, there is an increase in the proportion of overweight and obese people among the population, which contributes to an increase in the number of socially significant diseases. Obesity is associated with the predominance of the proportion of fat mass in the component composition of the body. Based on this, increased attention is paid to a comprehensive study of the functioning of all types of human adipose tissue, which is necessary for the further use of this knowledge in the fight against obesity. This article provides an overview of the literature data on the structure and properties of the PRDM16 gene, the features of its functioning in brown, white and beige adipose tissue. The influence of the PRDM16 gene on the activation of brown adipogenesis and the formation of beige adipocytes in white adipose tissue, which contribute to increased heat production of cells by means of non-contractile thermogenesis and improvement of metabolic parameters of the body, is considered. Data on the role of PRDM16 protein in the treatment of obesity, type 2 diabetes mellitus and other human diseases associated with it are analyzed. The results on the methods of effective activation of brown and beige adipocytes and the use of these methods in the treatment of obesity are presented.

The global prevalence of hypertension and obesity continues to rise, affecting increasingly young people. Obesity causes hypertension through a variety of mechanisms, including sympathetic nervous system activation, renin angiotensin aldosterone system, fluid and electrolyte dysregulation, inflammation, and adipokine imbalance. In turn, arterial hypertension can exacerbate obesity by altering metabolic pathways and increasing appetite. The pathophysiological features of hypertension are different between young overweight women and men. We performed a non-systematic literature review to thoroughly investigate mechanisms of pathogenetic interaction and mutual aggravation of high blood pressure and body mass index. The literature was reviewed from 2004 to the present in Russian and English using the PubMed Central, ScienceDirect, Google Scholar platforms, as well as a search in the archives of the journals Circulation and Cardiovascular Therapy and Prevention using the keywords listed below.
