Is COVID-19-associated cytokine storm distinct from non-COVID-19 secondary hemophagocytic lymphohistiocytosis?

Abstract: Cytokine storm is an umbrella term that describes an inflammatory syndrome characterized by elevated levels of circulating cytokines and hyperactivation of innate and/or adaptive immune cells. One type of cytokine storm is hemophagocytic lymphohistiocytosis (HLH), which can be either primary or secondary. Severe COVID-19-associated pneumonia and acute respiratory distress syndrome (ARDS) can also lead to cytokine storm/cytokine release syndrome (CS/CRS) and, more rarely, meet criteria for the diagnosis of seco… Show more

“…Our patient had a significant increase in serum urea and creatinine levels when he was admitted to the hospital, but an improvement was seen with no need for dialysis. We suspected an immune-mediated mechanism or hemophagocytic syndrome; 27 however, the investigation was unremarkable, and the true etiology of renal damage was unclear.…”

“…In recent years, since the start of the COVID-19 pandemic, cases of SARS-CoV-2-induced secondary HLH during and post-infection have been sparsely reported [10][11][12][13][14][15]. In these case reports, the cause of HLH has been postulated to be secondary to subclinical inflammation and dysregulation of the immune system in patients of a wide age range with PCR-proven recovery [15][16][17].…”

“…В качестве основной причины такого несоответствия критериям выделяют неполноценное обследование (например, трепанобиопсия костного мозга, пункционная биопсия печени была доступна к проведению только в 30 % случаев) [17]. В связи с этим ряд авторов рекомендует воздержаться от применения H-score в диагностике ГЛГ у пациентов после перенесенной инфекции COVID-19 ввиду недостаточной чувствительности метода и в первую очередь опираться на клиническую картину [13,17,25].…”
