Is Early Gastric Cancer, Diffuse Type, a Forerunner of Advanced Gastric Cancer?

Abstract: The present study offers an original survey on GC in a defined Italian population. As far as the intestinal histotype is concerned, the slight age difference between EGC and AGC suggests that these tumors are different steps of the same process. On the contrary, the age distribution suggests that EGC, diffuse type, has a different biological behaviour.

“…The early stage has been attributed to alterations in DNA caused by chronic inflammation, an imbalance between epithelial-cell proliferation and apoptosis, and gastric colonization by enteric bacteria with nitrate reductase activity, which facilitates the formation of carcinogenic nitrosamines in a milieu of atrophy and achlorhydria. It may take many decades for normal gastric mucosa to progress to chronic gastritis, gastric atrophy, intestinal metaplasia, and finally to early gastric cancer (23,24), and it may take an additional 8 years for the early gastric cancer to become advanced gastric cancer (25,26). One of the functions of the p53 protein in a cell exposed to DNA-damaging agents is to induce G 1 arrest to allow the cell machinery to repair DNA damage or, in case of failure, to induce apoptosis.…”

“…This is only marginally older than the mean age in Japan (57.8 years; 3378 cases; three case reports); a surprising finding if one considers their screening programme. EGC probably occurs in younger adults than AGC; two series report a difference of eight years,8 23 a third of 23 years (though only five cases of EGC),35 whereas a fourth found no difference 6…”
