Is Esophagectomy for Benign Conditions Benign?

Abstract: Benign esophagectomies are associated with significant morbidity. Although the site of the anastomosis does not alter outcomes, use of colon/intestine conduit should be pursued with caution.

“…This limitation can be addressed by studying outcomes of ERAS implementation on benign esophageal resections which are known to have higher incidence of Clavien-Dindo grade IV complications. (25% vs 20%, p=0.003 and prolonged hospital stay in contrast to esophageal cancer surgery as described by Masabni et al 23 Multicenter trials with larger sample size are required for further validation of the results described in this study.…”

“…All 17 patients had interventions prior to esophagectomy, including myotomy and all had persistent or recurrent dysphagia. The QOL at a mean follow-up of 5.5 years was lower than historic controls for either laparoscopic Foregut 2 (3) heller myotomy or esophagectomy for malignant disease. Surprisingly the major culprit for the poor QOL was reflux (regurgitation), dry mouth, and trouble with coughing.…”

“…In the short term, it has been shown that the majority of esophagectomies for benign disease are associated with significant morbidity with a longer length of hospital stay and an increased rate of grade 4 Clavien-Dindo complications. 3 The largest review of long-term outcomes following esophagectomy for benign disease was performed by Bonavani et al In this series they identified 1307 patients that underwent an esophagectomy for achalasia. 4 They note that the majority of patients were able to eat a normal diet on long-term follow-up.…”

“…Следует отметить, что выполнение такого обширного и травматичного вмешательства, как ЭЭК, сопровождается достаточно высокой частотой как послеоперационных осложнений, так и летальности, которая, по данным различных авторов, может достигать 4-8% и более [54], следовательно, учитывая доброкачественный характер заболевания, при принятии решения о выполнении ЭЭК у таких пациентов необходимо тщательно оценивать соотношения «риск-польза».…”
